Public Contract

This public agreement (the Agreement) is an offer by the Municipal Non-Commercial Enterprise «Children's Medical Rehabilitation and Palliative Care Center» of Zhytomyr Regional Council (the Center) for the provision of medical, rehabilitation and palliative services. Applying to the Center for services constitutes full and unconditional acceptance of this Agreement.

The Center undertakes to provide medical, rehabilitation and palliative services to children in accordance with established medical standards and NHSU packages. The list of free services under packages №23, 24, 25, 53, 54, 86 is available on this website and at the registration desk.

Services covered by NHSU packages are provided free of charge. The list of paid services and the current price list are available on the Paid Services page. Payment is made through the Center's cashier or by bank transfer. The Center does not require advance payment for free NHSU services.

The Center undertakes to provide high-quality services, maintain medical confidentiality, and respect the rights of patients and their legal representatives. The Patient/Legal Representative is obligated to provide accurate medical information, comply with the Center's internal rules, and notify about appointment cancellations at least 24 hours in advance.

The Center guarantees the protection of personal data in accordance with Ukrainian legislation. Medical information is confidential and shall not be disclosed to third parties except as required by law. Personal data is processed solely for the purpose of providing medical services.

The Center is liable for the quality of medical services in accordance with applicable law. The Center is not liable for consequences arising from inaccurate information provided by the Patient. Disputes shall be resolved through negotiations, and if not possible, through the courts.

The Agreement takes effect upon the Patient's first visit to the Center and remains in force until services are completed. The Center reserves the right to amend the Agreement by publishing an updated version on this website. The current version is always available at this link.
